Before you come in, please print the intake form, fill it out and bring it with you to your first appointment.

What to wear? 

A gown is provided to female clients before each session; however, clients are welcomed to bring or wear comfortable clothing, such as athletic or loose wear.

Cancellation Policy

• Rescheduling may be made at least 24 hours prior to your appointment.

• Cancellations within 24 hours of your appointment will be subject to the rate of service.
